Owner Surrender Request

I hereby relinquish all rights of ownership, custodianship, and privacy rights of the above-described animal to the Humane Society of the Pikes Peak Region (HSPPR). I am the rightful owner, have complete responsibility for this animal, am authorized to surrender this animal, and have the authority to act on behalf of this animal. By relinquishing this animal, I understand and agree that HSPPR may do a thorough and complete examination of the above-described animal as deemed necessary. In situations involving euthanasia, this may include a post-mortem forensic examination administered by a licensed veterinarian. I acknowledge that HSPPR cannot guarantee the outcome for this animal including if it will be made available for adoption or its length of stay at the shelter. Euthanasia is a possibility and HSPPR is not required to notify me if that is the outcome.
